Position: Certified Nurse Practitioner

Position Status: Full Time

Department: TCC Health

Location: Portsmouth and Franklin Furnace

Job Summary

The Certified Nurse Practitioner is responsible for providing direct health care services to patients in collaboration with the consulting physician.

Essential Functions

Essential functions are duties which are essential or primary to the position or the reason the position exists. An individual must be able to perform the essential functions of the position with or without reasonable accommodation.

The Certified Nurse Practitioner performs an integral role in carrying out the mission of The Counseling Center by performing the following duties.

Patient Care

Conducts comprehensive or episodic health assessments.

Develops the patient plan of care within clinical nursing practice and medical care protocols.

Appropriately prescribes medications as indicated.

Evaluates outcomes of care delivered to patients and initiates changes in the plan of care accordingly.

Assumes accountability for patient care in making decisions, delegating tasks, and maintaining a comfortable environment for the patient.
